Directions: On Main Street, turn right uphill at Firestone Tire. We are one block above Firestone, on the corner. We’re at the 4-way stop where Washington Avenue and 5th Street meet, in a big 3 story brown brick building.
Parking: There is free parking for our patients in the Private Lot on the corner of 5th Street and Lee Avenue. This lot is located directly across the street from the side of our building, to the east. The lot has a large white sign: “PRIVATE PARKING”.

Updated Notice of Health Insurance Portability and Accountability Act (HIPAA) Privacy Practices As a HIPAA-compliant office, patient health information is kept confidential and secure at all times. In addition to our regular protocols, protected health information (PHI) is not used by this office for fund-raising or marketing without written authorization. Patients will be notified immediately if a breach of information security occurs. If a patient pays for a service out of pocket, he or she has a right to request that their treatment information not be disclosed to their health plan. All patients in this office are notified that they have a right to a copy of their health records, as per Federal HIPAA Omnibus Rule, January 25, 2013, effective September 23, 2013.
This office is (ADA) Americans with Disabilities Act (Title III) compliant. As per the Federal rule effective April 27, 2014, protocols are in place to make reasonable accommodation for members of the public with disabilities. Equal accessibility of dental care within our facility is provided via office policies, practices and procedures which implement reasonable accessibility. Individuals in WV with a Hearing/Speech disability are encouraged to speak with us through the WVR, which enables specially trained communications assistants to act as HIPAA compliant bridges between our office and hearing impaired individuals. Those with Vision disabilities are encouraged to use their operator assisted TTY/TDD systems to contact us.
